The HKGIA Technical Consultant (Hong Kong Productivity Council, HKPC) has adopted a Personal Data (Privacy) Policy. You may contact HKGIA Technical Consultant to request access to, and amend your personal data provided by you. If needed, please send an email to: sec@hkaee.gov.hk. The personal data collected from you will be erased and destroyed after 24 months upon the completion of the assessment of HKGIA.

本人謹此聲明以上提供之資料全屬確實無訛，亦同意「香港綠色創新大獎」主辦機構（環境及生態局及環境運動委員會及其秘書處）及評審團所作的一切決定為最終決定，並於一切有關「香港綠色創新大獎」事宜上均具約束力。本人明白任何錯誤或誤導的資訊或會令本人的申請被取消資格。

I hereby declare that the information given above is accurate to the best of my knowledge, and agree that all decisions made by the Organisers (i.e. Environment and Ecology Bureau and Environmental Campaign Committee and its Secretariat) and adjudicating panel are final and binding in all aspects relating to the HKGIA. I understand that any false or misleading information may lead to disqualification of my application.

本人同意本人所提供之個人資料（包括姓名、聯絡電話、通訊地址和電郵地址）將會用於主辦機構及技術顧問作活動聯絡、處理、評估和管理本人申請之用。本人明白如未能提供相關個人資料，會影響主辦機構及技術顧問處理本人之申請。
